  • An Eligibility extension is required if the eligibility determination is not able to be completed and entered into QE2 within 60 days of the date of application; 
  • Eligibility extensions not completed and entered into QE2 before the 60th day, are out of compliance;
  • All Eligibility extensions require client agreement and Office Director approval;
  • Only one extensionup to an additional 60 days, is permitted per case for a maximum of 120 days (4 months); whether the Extension is 60 days or less than 60 days, only one extension can be completed;
  • When an eligibility determination is not completed by the agreed upon extension date, staff have 3 working days to respond to the autogenerated email sent to the Specialist, Office Director, and Director of Nebraska VR
  • Cases not made eligible prior to the agreed upon extension date require Office Director approval.

An Eligibility Extension is required if the eligibility determination is not completed and approved in QE2 within 60 days of the date of application.

  1. Prior to the 60 day Eligibility due date, discuss the need for an eligibility extension with the client. Explain exactly what caused the delay, what is needed to complete the eligibility determination and why.
  2. Outline the specific steps and timelines required for the Eligibility approval within the agreed upon extension date.
  3. Obtain agreement based on informed choice with the client for the extension and estimated date it will be completed. The client needs to agree to the extension for the case to remain open. 
  4. Complete the Eligibility Extension screen in QE2. Include why the extension is needed and the specific steps to be taken by VR and the client to complete the eligibility by the agreed upon extension date. 
  5. Only one Eligibility extension is permitted, per case for a maximum of an additional 60 days or a total of 120 days. Whether the extension is 60 additional days or less than 60 days, only one extension is allowed. 
  6. Office Director approves the Eligibility extension in QE2 indicating agreement to the steps and timelines outlined in the extension. A task note will be automatically generated when the extension is approved. 
  7. The Office Director is responsible for monitoring all extensions to ensure the specific steps outlined are being met and the eligibility is completed as soon as possible, but no later than the agreed upon date. Every effort should be made to approve the eligibility prior to the agreed upon extension date. 
  8. In the rare occurrence when the eligibility determination is not completed by the agreed upon extension date, an automated email will be sent to the Specialist, Office Director, and Director of Nebraska VR. The VR Specialist must task note in QE2 AND respond to the email within 3 working days with: 
    • an explanation of why the eligibility was not completed by the agreed upon date; 
    • the specific steps and timelines for either completing the eligibility or closing the case 

    9. The VR Office Director must monitor the specific steps and timelines outlined in the email to ensure the VR Specialist completes the eligibility or closes the case, while also ensuring all policy and procedures are followed.

    10. Cases not made eligible prior to the agreed upon extension date requires an Office Director approval.

If 60 days have expired without an Eligibility Extension being completed -

  1. An automated e-mail will be sent to the VR Specialist, Office Director, and Director of Nebraska VR.
  2. The VR Specialist must task note in QE2 AND respond to the email within 3 working days with: 
  • an explanation of why an Eligibility extension was not completed and entered into QE2 within 60 days of the date of application; and 
  • the next steps for either obtaining client agreement, completing the extension and determining eligibility or closing the case. 
  1. The Office Director must follow up on the specific steps and timelines outlined in the email to ensure the VR Specialist completes an extension and determines eligibility or closes the case.
