• for approval of non-employee (student intern, volunteer, student worker, practicum student, SOS or other individual from outside the agency)

• completed Request Temporary Access form

2. Emails Supervisor with approval or disapproval of non-employee.

3. Emails approved Request Temporary Access form to:

• Laura Stueck
• Victoria Rasmussen
• Supervisor

4. Discusses the following VRIS Program Manual Chapters with the non-employee:

• Records, Confidentiality and Information Release
• Confidential Electronic Client Information

5. Discusses Administrative Memorandum #503, Volunteers and Unpaid Interns
•Not required for SOS workers

6. Prints the:

• Confidentiality Agreement
• NDE Volunteer/Unpaid Intern
• Background Screen Release of Information (Disclosure and Authorization Form)

7. Completes the:

• Confidentiality Agreement
• NDE Volunteer/Unpaid Intern
• Background Screen Release of Information (Disclosure and Authorization Form)

Note: SOS workers do not complete the:
• NDE Volunteer/Unpaid Intern
• Background Screen Release of Information (Disclosure and Authorization Form)

8. Provides non-employee with a copy of the Confidentiality Agreement and retains original.

9. Scans and emails the following form to the VR Assistant Director, victoria.rasmussen@nebraska.gov:
• Confidentiality Agreement

10. Scans and emails the following forms to the VR Director:
• NDE Volunteer/Unpaid Intern
• Background Screen Release of Information (Disclosure and Authorization Form)

11. Prints and signs the NDE Volunteer/Unpaid Intern form.

12. Submits the following approved forms to NDE HR with “cc” to Supervisor:
• NDE Volunteer/Unpaid Intern
• Background Screen Release of Information (Disclosure and Authorization Form)

 

 

 

 

 

 

Any non-employee (student intern, volunteer, student worker, practicum student, SOS worker [also follow Program Manual chapter: SOS-Specialized Office Services Process] or other individual from outside the agency) who may have access to confidential client information or have contact with Nebraska VR clients must be oriented to the agency policies on confidentiality and complete a background screen.

  • • Background screening is not necessary if the non-employee will not be accessing confidential client information or having direct contact with clients of Nebraska VR.

Access to Outlook, Adium, QE2 and/or VRIS must be requested and approved by the VR Director by emailing a completed Request Temporary Access form.

The individual may begin performing duties before the background screen is completed unless Nebraska VR is aware the applicant has reported any conviction (not just felonies).
