- If registering for the first time, the individual must provide a copy of the following;
- Current and valid photo ID showing the same name and residence address provided on the Voter Registration form, or
- A copy of a utility bill, bank statement, government check, paycheck or other government document which is dated 60 days immediately prior the date of presentation showing the same name and residence address provided on the Voter Registration form.
- If the individual assumes responsibility for submitting the Voter Registration Form, the individual should mail or deliver the form to the election commissioner or county clerk of the county the individual resides in.
- If the individual provides the form and identity/residence copy to VR to submit, the form must be submitted no later than 10 days from the date of receipt to the election commissioner or county clerk of the county in which the VR office is located. Then the election commissioner or county clerk's office will forward if the individual does not reside in the same county as the VR office.
- If the date of receipt is within 5 days prior to the third Friday preceding any election, the form shall be submitted no later than 5 days from receipt.
