Summary of updates:
The DHHS Information Release Form for Background Screens has been updated by DHHS. Old forms will no longer be accepted by DHHS for Registry Checks effective March 1st.
PLEASE NOTE: There is an option of Section A or B to be filled out about the identification of the applicant. Complete Section B which allows for NDE signature to avoid the use of a notary.
