In the Program Manual, the organizational structure of the Pre-Employment Transition Services Chapter has been changed, along with some pertinent information. A VR Services in Transition section has been added under Case Services. There are times when a student, aged 14-21, will request or require individualized transition and other VR services, where an application and eligibility determination will need to be in place in order to provide these services.

In the Program Manual, a Pre-Employment Transition Services section has been added to Forms. Here you will find all of the forms pertinent to Work Based Learning Experiences. Please note the different requirements on the Consent for a Paid Work Based Learning Experience based on age, 14-15 yr. old and 16+.

Definition of an Educational Program: Educational Program is a term used to include secondary education programs, non-traditional or alternative secondary education programs, home schooling, post-secondary education programs, GED, vocational/technical programs, short term training programs such as CNA, and other recognized educational programs. Educational programs should lead to a recognized credential of education.

Please note Allowable Supports for Pre-Employment Transition Services:

The Service Agreement form has been updated to include Worksite Skills Trainer.
