• Job Coach must be at least 19 years of age; 
• Job Coach must supply his/her own tools and uniform to carry out the job coaching responsibilities;
• Language regarding training and experience (under “Agency or Business Provided Job Coach”); and
• Specifications related to Transportation, Cancellation of Service, Job Coach Responsibilities, and Termination.
