Summary of Changes
NE DHHS is also now requiring a new Release of information that must be signed by the person (on the first and second pages.). The form is filled out with pre-populated information so that it is completed consistently, noting the information to be released, except that the name of the Benefits Services provider must be filled in so that information will be released to that agency. There are English and Spanish versions of the form.

Ensure that the new Release of Information is used so that information can be released by DHSS to the Benefits Services agency. Note that the form must be signed in two places--p. 1 and p. 2.
