Summary of Changes
NOTE these changes are effective 12/1/18.
1) Referrals and authorizations for Benefits Assessment service to be completed for all people prior to writing of IPE.
2) Referrals for Benefits Planning service to be completed for all clients after IPE is written.
3) Process for documenting choice of provider or refusal of services is outlined.
4) All services changed from an hourly rate to a flat fee for service.
5) Critical touchpoints and other circumstances when Benefits Management may be authorized are described. Allows for one authorization of Benefits Management while case is open; one authorization for Benefits Management in Post-Employment.

Ensure all staff attended Benefits Services training held in November. Contact C. Lasley if additional sessions are needed after 12/1.
Revised service agreement effective 12/1/18. OD's to receive copy after these are executed.
Team Communication
Discuss details of changes in team meetings as staff are trained and become familiar with new process; convey process questions and concerns to C. Lasley for resolution.
