Summary of Changes

NOTE that this change is effective 1/1/19, and revised forms will be attached to the associated chapter and available in Forms as of that date.  This update is sent to alert you to upcoming changes.  There is NO change to the chapter at this time.

Changes to the Payment schedules and Milestone 5 A/B forms were reviewed during the Service Agreement training held on 11/26/18. The Independent Provider packet includes all forms specific to this type of  I/DD SE/CE provider.  Changes to the previous Independent Provider packet are as follows:

• The Overview of NE VR's SE Services was revised to describe activities performed under each milestone; changed the description of Milestone 5 to reflect the milestone payment at 6 mos and 12 months (MS 5A and 5B)
• The term "consumer" was removed throughout in favor of the term "client
• The Milestone Payment Schedules for SE and CE were revised to reflect the change to add the description and payment provisions for the Discovery service and for the revised Milestone 5 (now Milestone 5A and 5B)
• A form to report activities under Milestone 5A and 5B replaces the former Milestone 5 form
• The Independent Provider Billing Document was revised to include Discovery and Milestone 5A/5B payments categories

Forms - New or Revised

Independent Provider Packet -- forms changed are:  Overview, Definition of Terms, SE I/DD Payment Schedule, CE I/DD Payment Schedule, Milestone 5 A/B, Independent Provider Billing Document.

QE2 Update

QE2 changes are pending to authorize Milestone 5 in two payments: Milestones 5A and 5B ($500) at 6 and 12 months, respectively, when the person remains employed in the same job as at closure.

Review changes at upcoming team meetings; ensure that this updated packet is in use effective 1/1/19.
