Summary of Changes

NOTE that this change is effective 1/1/19, and revised forms will be attached to the associated chapter and available in Forms as of that date.  This update is sent to alert you to upcoming changes.  There is NO change to the chapter at this time.

Changes to the Payment schedules and Milestone 5 A/B forms were reviewed during the Service Agreement training held on 11/26/18, and are effective 1/1/19.  The changes are as follows:

• Milestone Payment Schedules for I/DD Supported Employment, I/DD Customized Employment and ABI/Autism Supported and Customized Employment were revised to add the description and payment provisions for the Discovery service and for the revised Milestone 5 (now Milestone 5A and 5B)
• Milestone 5A / 5B Form replaces the former Milestone 5 form, reflecting that a report is due at 6 months and 12 months after closure when the person retains the same job held at closure
• The Milestone 5A / 5B form will now be used for to report milestone activities for those with I/DD, ABI, Autism, and the name of the form has been changed to reflect that
• BH SE forms changes will be made at a later date in collaboration with DBH; forms currently posted should remain in use until a separate update is sent     
• Customized Employment was added to the Payment Schedule for ABI/Autism, reflecting the option of developing a customized job for those with brain injury or autism.  )The payment rate for each milestone is the same for customized employment as for traditional supported employment)

Forms - New or Revised

Milestone 5 A/B; Payment Schedules, as noted above 

QE2 Update

QE2 changes are pending to authorize Milestone 5 in two payments: Milestones 5A and 5B ($500) at 6 and 12 months, respectively, when the person remains employed in the same job as at closure.

Review all changes in a staff meeting; remove any paper or saved copies of forms and begin using updated forms.
