(This form is no longer in use by DHHS, as they are now accepting only the release of information form titled: DHHS Authorization for Disclosure of Protected Health Information-- HHS 160 -- statewide. Some offices were still requesting both for a time.)
2) Posted Title II: Social Security Disability Income Work Incentives Guide - 2019
3) Posted Title XVI: Supplemental Security Income Work Incentives Guide - 2019 (note that the SGA will not be updated until later in the year so that will be updated when change is received.)
