Summary of Changes

Added:

• Court appointed guardian & SSI/SSDI benefits information
• If an individual is unable to be seen within 10 calendar days of the referral, a TASK notes needs to explain the delay.

Forms - New or Revised

Updated Referral form:

• Clarified SS# not required at time of referral
• Added more contact information for contacts
• Changed high school section from year in school to graduation year.
• Removed NDE Student ID and current benefits

QE2 Update

If individual requires communication in Spanish, in Qe2 check the "Spanish communication" box, under client's personal information. This will serve as a reminder for the need of an interpreter and written communication to be sent in Spanish.
