Summary of Changes

Clarified required forms and providing the applicant a copy of documents completed.

Revised the form name, added demographic data to collect per federal requirements, and revised instructions for entering data from Nebraska VR ABI Interview Form in QE2. 

Updated chapter references to the Nebraska VR ABI Interview Form and interview process. Added a link to the Brain Injury Advisory Council website in the chapter for staff to download an ABI resource brochure for applicants.

Deleted the outdated Nebraska VR Brain Injury Information, Referral and Resource Facilitation Services Brochure.

Team Communication

Please use up existing VR Applications first.

Begin using new Nebraska VR ABI Interview Form and entering data in QE2 on February 15, 2022. Destroy any old, printed ABI Screen forms after that date. Until February 15, continue sending completed ABI Screen forms to Keri Bennett at keri.bennett@nebraska.gov After February 15, 2022 staff can discontinue sending completed forms to Keri Bennett as all data will be entered in QE2.

Monitoring Requirements

Case Reviews

If the application pre-screening question is answered with “yes” or “not sure”, an ABI Interview should be completed and the results entered in QE2.

 

Training Dates

Nebraska VR ABI Interview training dates are: Tuesday, February 15 at 2:00 pm, and Friday, February 18 at 10:00 am Central time.

 

QE2 Update

Nebraska VR ABI Interview Form answers may be documented directly in QE2 or on the form for input in QE2 later. ABI Interview results are summarized on a separate screen, and a flag is placed on the Functional Capacities screen in QE2 to remind staff to review the Interview results and summary when determining functional capacities.
