Summary of Changes

Revised process for determining end of MS 4 (when client is ready to transition to extended services), and 90 day period for transition to extended services.  

Provided directions for closure of BH SE cases and how other SE cases are handled following MS 4.

Add guidance developed some time ago and decision-tree to support staff in determining when SE is an appropriate goal and how to identify possible sources for extended services.

Forms - New or Revised

Revised MS 4 forms -- now titled Transition to Extended services and includes description of services and supports to be provided during transition.

Revised Milestones payment forms to describe MS 4 process, MS 5A/B applicable changes.

 

QE2 Update

Change coincides with other changes to QE2 related to IPE.   

 

To Do

Follow new process for MS 4 for applicable SE type (BH, or I/DD, ABI, Autism); use guidance as needed when developing SE plan and projecting possible sources of extended services.

Team Communication

Discuss / review policy changes and seek resolution of any issues, answer to questions, if applicable.

Monitoring Requirements

No specific monitoring required, other than typical review of cases. 

 

Training Dates

MS 4 changes covered at trainings on 3/ 16 and 3/18. Will also review changes at 4/14 and 4/15 follow-up IPE meetings.
