As a result of the IPE Project, information and referral provided during CCIR meetings must now be documented differently. See PROCESS section #5 of the chapter:
5. Individual is provided an Information and Referral (I & R) to Other Programs handout, edited specifically for each person's unique situation (paper or e-version acceptable). Information & Referral discussed during CCIR meeting must also be documented in QE2. From Information & Referral service, select +Document Team Service to summarize referral sources provided.
