Added S&D application and VR team meeting with ATP to Process section. 

Added process of how to authorize: 

• Driving Evaluation: If the client does not have medical insurance to cover the cost of the evaluation, authorize: Driving Assessment: Career Planning - Other Assistance (assessment support service) 
• VR submits authorization to ATP as an IPE Service: Rehab Technology - Vehicle Modification - New 
• If driver's training is needed, Authorize as an IPE Service: Driver’s Training: Disability Related Skills Training to vender.
