Summary of Changes

The end date of the Job Search and Placement service (milestone 2) has been changed from the date of the job offer to the job start date.  

Updated guidance on the accepted form(s) of verification for client hours worked:  Accepted forms of verification are: 1) Verification of Employee Hours Worked form, signed by the client and the employer; OR 2) Documentation from the place of employment (e.g. paycheck stub, employee timesheet, email, etc.) that includes the client's name (pre-printed or signature) and the number of hours worked per day.  If the documentation is not from the place of employment (e.g. client wrote down their hours), an employer signature is required.  *In the event the provider is unable to secure documentation that meets the above requirements, contact the Program Director for Supported Employment to discuss an exception.  

The Supported and Customized Employment Fiscal Chart has been updated to reflect the changes above.  The "Report Received Stamped on Invoice" requirement has also been removed.  The VR specialist or office director confirms that the SE service has been completed by signing the SE invoice.

QE2 Update

The standard text in the Job Coaching and Support and Extended Services authorizations has been updated to reflect the changes in accepted forms of verification.
