Summary of Changes

The Initial Meeting chapter was updated to reference the new “Nebraska VR Guide – Tools for Assessing Cognitive Function”. The guide is intended to help staff find and use the right tool for assessing cognitive impairment for clients with acquired brain injury (ABI) or other cognitive disabilities. The guide includes a description of the Functional Cognitive Assessment, which is a new Assessment Support service available in QE2 as of March 1, 2024. A description of this new service is also included in the Assessment Supports Form associated with the Assessment Supports chapter on VRIS.

Forms - New or Revised

The new “Nebraska VR Guide – Tools for Assessing Cognitive Function” will be available on VRIS under Guides & Resources, Job Planning Resource Guide. It replaces the “Comprehensive Neuropsychological Evaluation Guide” which will be deleted from VRIS. A new form titled "Functional Cognitive Assessment Referral" was added to the VRIS Case Services - Application, Eligibility, Planning forms section to accompany the new service listed in the Assessment Supports Form. When referring a client for this service, please complete and send the "Functional Cognitive Assessment Referral" form to the provider with the authorization for the Functional Cognitive Assessment service, along with a signed release form and any re-releasable records as noted in the guide.

QE2 Update

A new Assessment Support service is available in QE2 titled “Functional Cognitive Assessment”. The new guide includes a description of this service and instructions for referral and authorization.
