Summary of Changes
Goodwill Columbus has changed its addresss for the office providingn benefit services. With these changes (suite # and phone number) forms were also revised to be 508 compliant, meeting accessiblity requirements. These forms are the BP/SSA 3288 Consent for Release of Information (English and Spanish) and BP/DHHS Authorization for the Disclosure of Protected Health Information (English and Spanish). Radio buttons have been replaced by a dropdown list to select the provider to which information may be released. As before, you must use Chrome to make the form functional.
