Amend IPE Job Goal - Prior to Client accepting a job
Complete an IPE Job Goal/Supported Employment Goal and applicable IPE services forms when you and the client have agreed to a new job goal the client will pursue. (For Supported Employment cases, only needed Service forms, if a service or support is being provided that is not within the scope of supported employment).
Amend IPE Job Goal at Outcome
Complete the IPE Amendment of Job Goal at Outcome and IPE Amendment Termination of Services form.
Complete the IPE Amendment of Job Goal at Outcome and IPE Amendment Termination of Services form.
IPE amendments are required when a ‘substantive change’ is made in the employment goal, VR services or in the provider of services.
The IPE must be amended when:
